When a woman is trying to conceive, there are many things that can impact her chances of getting pregnant. For some women, certain treatments can help them conceive. These include lifestyle changes, medication, hormone treatment and assisted reproductive technology (ART). In some cases, surgical procedures are necessary. A medical evaluation is a key first step in fertility treatment. A gynecologist, urologist or family doctor can evaluate your health to determine if there are any factors that could be impacting your ability to conceive.

During a medical evaluation, doctors will take a detailed history of your symptoms and family health history. They will also perform a physical exam of the pelvis and abdomen. They may feel for abnormalities in the fallopian tubes and uterus and check for blocked or scar tissue in the uterine cavity.

After a medical evaluation, your doctor will recommend an infertility treatment plan. Treatments will depend on the cause of the infertility and your goals. For example, a surgical procedure is often recommended for endometriosis or uterine fibroids. A hysteroscopy is used to identify and remove abnormalities in the uterine cavity, including polyps, fibroid tumors or scar tissue. The surgery is performed through a small incision in the abdomen, similar to a Cesarean section. It is usually done in the hospital on an outpatient basis.

Fertility medication is often used to stimulate egg production in women. The eggs are then fertilized in the laboratory for IVF.

Medications can also be used to treat male infertility. They can improve sperm count and motility and increase the chances of fertilization with an egg. Sperm can be collected with IUI or via artificial insemination (IA). Medications can also be used to treat low or high levels of certain hormones, which can impact fertility.

If a woman has a condition such as polycystic ovary syndrome, a minimally invasive surgical procedure called laparoscopic ovarian drilling can be performed to help stimulate ovulation and increase your chances of becoming pregnant. This is a minimally invasive procedure that uses an ultrasound guide to locate the follicles and a thin needle to extract the mature egg for use in IVF or intracytoplasmic sperm injection (ICSI).

An oocyte transfer is done when the eggs are combined with sperm in a lab to form embryos. After the embryos have ripened and grown for three to five days, they are transferred to the uterus.

For IVF, a procedure called assisted hatching can aid the implantation of the embryo by opening the outer layer of the embryo to allow sperm to penetrate.
